HealthTrack SG

HealthTrack SG is the Health Promotion Board's year-long digital health programme and pilot for selected Singapore residents with chronic conditions or related risk factors. It combines wearable-device information, self-reported information and clinical information to support a personalised Wellness Plan and a Wellness Score (HPB, accessed 29 Aug 2026; HPB FAQ, accessed 29 Aug 2026). Its published terms describe programme information as general information rather than medical advice (HPB Terms and Conditions, accessed 29 Aug 2026).

Eligibility and registration status

The published eligibility profile is Singapore Citizens or Permanent Residents aged 25 to 75 who have one or more of hypertension or high-normal blood pressure, Type 2 Diabetes or pre-diabetes, lipid disorders or high cholesterol, BMI of at least 23, or waist circumference of at least 80 cm for females or 90 cm for males. A participant also needs a suitable smartphone and wearable, spoken and written English, and must be selected as suitable by HPB; meeting a listed criterion does not itself guarantee selection (HPB, updated 1 Apr 2026). Public registration closed on 31 March 2026 according to HPB's programme page, so current availability should be checked with HPB rather than inferred from the eligibility list (HPB, accessed 29 Aug 2026).

Participation workflow

Selected participants use the HealthTrack SG and compatible wearable applications, complete onboarding activities, attend an appointment with the appointed healthcare provider Fullerton Health, and complete blood tests. During the programme, they are expected to wear the device, report their health status in the app every two weeks, and engage regularly with the programme. HPB says that more than 90 days without wearable or Report Health Status data may result in automatic withdrawal (HPB FAQ, accessed 29 Aug 2026). The FAQ states that participants have at least an initial and final doctor visit; prevailing consultation and blood-test charges apply (HPB FAQ, accessed 29 Aug 2026).

Partners, rewards and nearby programmes

HPB administers HealthTrack SG with Google, ConnectedLife and Fullerton Health. The programme may provide a subsidised Fitbit or Google Pixel wearable to a selected participant who does not already have a compatible device (HPB, accessed 29 Aug 2026). HPB's published milestone table describes rewards totalling up to S$140 in eVouchers for onboarding, required tests and consultation, Report Health Status activity and programme completion; the amount depends on completion of the applicable milestones (HPB FAQ, accessed 29 Aug 2026).

HealthTrack SG is distinct from Healthier SG. HPB describes it as complementing Healthier SG, so participation in Healthier SG does not by itself prevent participation in HealthTrack SG (HPB FAQ, accessed 29 Aug 2026).
